Membership Application
To apply for
membership into VAISEF, please mail 3 completed copies of
the application along with the $100 application
fee to the
VAISEF office at 1011 East Main Street, Suite 400, Richmond,
VA 23219. Upon
receipt, it will be reviewed by the Accreditation and Membership
Committee and then passed on to the VAISEF Board of Directors
for final approval. From the date of submission of the
application to final approval by the Board of Directors usually
takes 60
to 90 days.
Accreditation Application
To apply for VAISEF
accreditation for your facility, please mail 3 completed
copies of the application, the
$100 application fee, and the appropriate site-visit fee to the
VAISEF office at 1011 East Main Street, Suite
400, Richmond, VA 23219. To
determine the appropriate site-visit fee, click on the link for
Costs Associated with Membership & Accreditation. Only
VAISEF members may be accredited through VAISEF. However,
facilities may apply for membership and accreditation simultaneously. From
the date of submission of the application to final approval by
the Board of Directors usually takes 60 to 90 days.
Re-Accreditation Application
To apply for
VAISEF re-accreditation for your facility, please mail 3 completed
copies of the re-accreditation application, the $100 application
fee, and the appropriate site-visit fee to the VAISEF office at
1011 East Main Street, Suite 400, Richmond, VA 23219. To determine
the appropriate site-visit fee, click on the link for Costs
